1. Hair Transplant Rejection

Hair transplant rejection can occur in some patients, leading to an unsuccessful surgery and disappointing results. Lichen planus (LLP) is a type of primary scarring alopecia that can cause graft rejection. It is characterized by inflammation of the hair follicle, which results in the replacement of the follicle with scar tissue. It is important for patients with lichen planus to take into account the potential for their body to reject the graft before deciding to undergo a hair transplant procedure.

 

2. Lack of donor hair

The outcome of your surgery is contingent upon both your overall health and the particular condition of your hair. A healthy and appropriate donor site is an indispensable prerequisite for successful hair transplantation.

Overlooking the health of the graft and the age of the donor site can be a critical mistake during a hair transplant, as they directly affect the outcome of the procedure. It is important to provide adequate care to the newly transplanted hair follicles, promote healing for both the donor and transplant sites, and minimize any potential damage to the hair follicles.

 

It’s not only medical professionals who can damage hair follicles during a transplant surgery; patients themselves can also cause harm. Therefore, it’s crucial to be cautious and avoid pulling out grafts for the first 7-10 days following the procedure. Hair follicles may fall out right after surgery and may take up to 10 days to grow back. To avoid losing hair follicles, refrain from scratching or bumping the graft site for at least 10 days. It’s best to avoid wearing tight hats or beanies right after surgery as they could cause problems with the graft site.

 

To prevent hair loss and swelling after surgery, adjust your sleeping position to avoid friction at the transplant site and avoid applying pressure or traction for the first few weeks. Proper aftercare is crucial for optimal hair transplant results, so it’s essential to steer clear of vigorous activity, alcohol consumption, hot or cold showers, and excessive sun exposure during the healing process.

 

5.Inexperienced clinic

 

Hair transplant failure can also be the result of choosing the wrong clinic. Since hair transplantation is a very popular surgery, many clinics and surgeons have started offering it. However, not all of these clinics are experienced and professional enough to provide great hair transplant results.

 

The lack of experience in some clinics can result in the extraction of incorrect donor grafts or excessive harvesting of the donor area, leading to suboptimal transplant outcomes and increased dropout rates. Ensuring the proper handling and storage of donor grafts during surgery is essential to prevent the negative effects of shock trauma and subsequent hair loss.

 

Even if the doctor performs the surgery with the best technique and the patient takes proper care after the surgery, the grafting rate may not be high.   1. The hair follicle should be extracted without damage with a personalized punch.

When it comes to hair transplants, engraftment is key. FUE hair transplant involves harvesting hair follicles from your own back of the head, and how I harvest the healthy grafts determines the engraftment rate. Typically, a tool called a “punch” of about 1 millimeter is used to harvest the follicles. Each person’s hair follicles are different in shape and thickness. so it’s important to use a punch with the right diameter to ensure that the follicle is extracted without damage.

A narrower punch may cut the follicle, while a wider one may unnecessarily remove a lot of surrounding tissue, resulting in a higher rate of scarring. It takes skill on the part of the practitioner to use the optimal punch for each individual.

 

  1. Maximizing the effect with fewer hair follicles.

The cost of a hair transplant varies from clinic to clinic, but it’s best to calculate on a per-follicle basis. Some people grow only one hair per follicle, and some grow two to four hairs in a single follicle. These are called multifollicular follicles, and harvesting and transplanting multifollicular follicles can increase the efficiency of the surgery.

In other words, instead of transplanting 1000 hairs from 1000 follicles, transplanting 1,000 hairs from 500 follicles is more effective. To achieve this, the surgeon must have the know-how to harvest and can extract healthy follicles without damaging them.

 

  1. Make sure to make slit sites in the direction of the original hair.

To transplant the harvested hair follicles into the balding area, a small hole called a “slit” is made. The idea is to create a home for the follicles so that they can grow well, and the size and depth of the slit is made according to the harvested hair follicles. Right size of slit ensures follicles to have good access to oxygen and nutrients, which will increase the grafting rate. Also, the slit should be made in the direction of the original hair so that the hair grows in its natural direction. The hair at the temples grows very close to the skin, so the slit should be made at a low angle.

On the crown of the head, the slit should be made in a whirling motion so that the result is natural. If the direction and angle of the slit is not right, it may cause side effects such as stretching of the grafts. The surgeon needs to know how to make slits according to the characteristics of the hair in each area.

 

     4.Make sure the grafts are intact and fit perfectly.

All of my hard work can go to waste if I harvest a healthy follicle and it doesn’t transplant well. Grafting the harvested follicles reliably is a very important step in determining engraftment rates. The slit hole is about the same size as the follicle or slightly smaller.

Therefore, small forceps are used to pick up the follicle and transplant it into the hole. It’s best to do this in a single pass with minimal manipulation. Too much manipulation of the follicles can damage them and affect their engraftment rate.

In order to ensure that the hair follicles are tightly interlocked with the tissue and are stable, the surgeon needs to be skilled and have an organized surgical system in place.

Both incisional and non-incisional can be customized to the patient. Which one is better will vary from patient to patient, so I will explain when FUE or FUT might be a better option.

 

Patients who benefit from doing FUT hair transplant

Those with a low volume of donor hair can benefit from doing FUT rather than FUE hair transplant. If you have a small amount of hair in the back, taking hairs one by one by FUE may result in further loss of density, which can make the back look hollow. In this case, it’s best to proceed with FUE hair transplant to avoid losing hair density.

Additionally, for patients with very curly hair, FUE method may cause damage to the harvested hair. In order to reduce the damage caused by the harvesting process, it is recommended to proceed with the FUT method.

 

 

Patients who benefit from doing FUE hair transplant

Those who have good hair density can benefit from doing FUE hair transplant. These patients are candidates for both FUE and FUT hair transplantation.   1. How should I manage transplanted hair? How long should I keep them moist?

The first 3 days after transplantation is a very important time. During this period, the transplanted hair is easily damaged, so you should avoid applying physical force such as touching or shock to the transplanted hair as much as possible.

 

  1. When can I start shampooing comfortably?

Shampooing is possible from the day after transplantation. During the engraftment period of 2 weeks, you should use foam shampoo instead of washing your hair as if you were massaging the scalp with your hands as usual. First, wet the entire scalp evenly using warm water with weak water pressure, put enough shampoo on the hand, rub it to create a bubble, put the foam on the transplanted area and leave it for 2-3 minutes, then carefully apply warm water with weak water pressure again. After 2 weeks, you can start shampooing comfortably.

 

  1. Does smoking and drinking alcohol result negatively in hair transplant outcome?

Alcohol and cigarettes cause inflammation. Therefore, the possibility of folliculitis in the hair transplant area increases when you drink alcohol or smoke. Not everyone develops folliculitis, but if folliculitis occurs, it is recommended to avoid alcohol and cigarettes as much as possible because the engraftment rate at the transplant site may decrease.

 

  1. When can start to exercise after surgery? Can I do light exercise right away?

Exercise should be avoided during the engraftment period of 2 weeks. The important thing is that you need to be careful not to sweat. But light exercise without sweating can be done during 2 weeks.

 

  1. Can I use hair loss treatment products like minoxidil right away?

Minoxidil should also be avoided for at least 3 weeks. Contact with minoxidil, especially at the transplant site, can cause irritation or inflammation.

 

 

  1. Do I need to take hair loss medicine after hair transplant?

Those who did not have hair loss but received hair transplant for hairline correction do not need to take hair loss medicine. However, those who have already had hair transplant and have undergone hair transplant must take hair loss medicine even after the transplant. Some people think that the transplanted hair will grow as it is right after hair transplant surgery. Today, I will explain the process of hair regrowth after hair transplant and you will be able to know what really happens to your transplanted hair.

Up to one month after hair transplant surgery is called the hair loss period, and the transplanted hair falls out in more than 99% of patients. For very selected people, transplanted hair will stay and will grow as it is. I have done more than few thousand cases, and I have only seen 3 cases where the transplanted hair came up as it was right after the surgery.

From 1 month to 4 months after surgery is called the hair latent period, and the transplanted hair will remain in a shed state. This is the time when patients are most anxious and express their dissatisfaction. Because they have gone through a surgery and nothing seemed to have changed yet. 

After 4 months, transplanted hair will start to grow out rapidly and we call this a hair growth period. During this periods, patients finally see changes on their hair state. When you take a look at the graph, 50% growth of transplanted hair occurs during 6 month periods and 90% growth of transplanted hair occurs within 12 months. So to really see the result of the hair transplant, you need to wait a year for an evaluation.

Is there no accompanying drop-off of the donor site for the FUE method?

in rare cases, accompanied loss of the harvested area and abnormal sensations may occur. Although FUE surgery is less likely to cause this than FUT surgery, it does happen sometimes.

 

If you think about doing 2nd and 3rd surgeries, is FUE method prohibited for the 1st surgery?

No, it does not matter and It is okay to perform the FUE for the 1st surgery. However, if you have low density of hair on the donor side, FUT might be a better option.

 

Is it better to do the FUE surgery if I am planning go to work tomorrow? When can I start to exercise? Can you resume with your daily life right after the surgery?

If you plan to go to work the very next day after surgery, it is best to do FUE. Because FUE does not cause enough pain so that it affects your daily activity. You can start exercising after 2 weeks. Because after 2 weeks, we believe engraftment of transplanted hair has occurred. You can resume with your daily life right after the surgery. I have seen people going oversees the very next day. You just need to be extra careful.

 

If I am worried about scars and pain, should I choose to do the FUE method? Is it still painful?

 If you are worried about scars and pain, FUE is your choice. It provides less pain, and less scarring. For the pain issue, there might be some itching and tingling sensation, but that’s about it.

 

How is anesthesia performed in the FUE?

In the same manner with the FUT. Every clinic has their own way of making anesthetic solutions, we use bupivacaine and lidocaine solutions.

 

How many hours does the FUE surgery take?

 It depends on the amount of hair follicles needed for the surgery. Normally, if a patient is going through 1500 graft surgery, it will take about 6~8 hours total. That includes preparing, acutual surgery time, and lunch break time as well.
